Clinical / Medical Services

TCA Health provides multiple services to help you and your family be well and live well within your community.

Behavioral Health

TCA Health knows that good health encompasses more than physical well-being. We offer social work services and referrals to help individuals and families cope with the many challenges life presents. We can provide comprehensive, psychosocial assessments for individuals and families and assist with referrals to community services, including mental health counseling and psychiatry. Our clinical social worker can help individuals and families work through school challenges, planning for college, marriage and family issues, depression, and peer pressure.

Care Coordination

Care coordination is a team-based approach to address the needs of patients with chronic diseases or individuals with high-risk conditions.  Our Care Coordinators help to bridge the gap between the patient and service providers and can help facilitate appropriate services, schedule appointments, assist with community resources, and encourage health promotion through patient education.


Oral health is important to your overall health. TCA’s general dentistry department provides a comprehensive range of dental services. These include diagnostic and preventive services such as exams, cleanings, fluoride treatments, sealants and x-rays; restorative services such as composite resin fillings and simple tooth extractions.

Internal / Family Medicine

TCA Health’s board certified physicians provide comprehensive primary care services to keep you and your family healthy. Our experienced and dedicated physicians, physician assistants, and family nurse practitioners provide quality health care to all of your family members from newborns to seniors.


TCA has a lab conveniently located on-site for any diagnostic blood or urine tests that may be required.

Obstetrics & Gynecology

We provide a full range of obstetrical and gynecology services for women in all stages of life. For emergencies, our staff physicians are on-call 24/7 at Advocate Trinity Hospital and Metro South Hospital. Our certified nurse midwife, a specialist in women’s health, provides obstetrical care for low-risk pregnant women.


Our pediatric staff strives to keep your children healthy and ensure they stay on track to meet all developmental milestones. We provide comprehensive pediatric services, including well child care, immunizations, and school physicals.


An on-site pharmacy provides a convenient place for TCA patients to pick up medications. Our pharmacist can assist with medication management and questions you may have about generic and other brands.  (Please note: Only established patients with TCA Health may fill prescriptions at the pharmacy.)

Ultrasound & Diagnostic Imaging

On-site  diagnostic imaging services on-site including x-rays, ultrasound, computed tomography (CT) scans, and magnetic resonance imaging (MRI).

Support Services

Health Insurance Outreach and Enrollment

TCA Health works to educate community residents about their health insurance options and enroll uninsured residents in qualified plans in the federal marketplace created by the Affordable Care Act, also known as Obamacare. Our Outreach and Enrollment initiative provides education about the available plans and assistance for residents who want to enroll and gain insurance coverage. Our specialists can help you determine your eligibility, explain the information you will need to have available when applying for coverage, and review the benefits and risks (if any) of the federal health insurance premium tax credit and cost-sharing program.

Health & Wellness Collaborative

TCA Health’s  Health & Wellness Collaborative promotes healthy lifestyle options and access to wellness activities for children and families of Chicago’s far south side and surrounding suburbs. The Collaborative offers fitness classes, nutrition education, and a community garden programming for youth, adults and children with the goal of reducing and preventing obesity and related conditions.

Supplemental Nutrition Assistance Program (SNAP)

SNAP (formerly Food Stamps), helps low-income people and families buy the food they need for good health. Benefits are provided on the Illinois Link Card, an electronic card that is accepted at most grocery stores. The program is managed by the Food and Nutrition Service (FNS) of the United States Department of Agriculture. and administered by the Department of Human Services (DHS). It  includes Employment and Training. As a partner with the DHS, TCA Health partners with local businesses and organizations to enhance participant’s employability skills that will support and assist them with gaining long term employment and assistance.

The Temporary Assistance for Needy Families (TANF)

TANF program provides temporary financial assistance for pregnant women and families with one or more dependent children. TANF provides financial assistance to help pay for food, shelter, utilities, and expenses other than medical.

Women, Infants & Children (WIC) Program  

The Special Supplemental Nutrition Program for Women, Infants, and Children (WIC) provides federal grants to States for supplemental foods, health care referrals, and nutrition education for low-income pregnant, breastfeeding, and non-breastfeeding postpartum women, and to infants and children up to age five who are found to be at nutritional risk. WIC helps parents to feed their children properly during critical periods of growth and development. The combination of nutrition education, nutritious foods, and yearly health care visits strengthens families long after their WIC eligibility has ended.

Other Services

Carver Military Academy School-Based Health Center

This high school based health center provides affordable, confidential, convenient, youth-centered primary and preventative health care for students attending Carver and who might otherwise not receive care. A family nurse practitioner and licensed clinical social worker provide primary medical and psychosocial services to all students whose parents have submitted signed consent forms. For parents or guardians, health care provided at school means they miss fewer days of work. For students, on-site health care can mean fewer missed days of school and improved mental and physical health. 

COVID-19 Services
TCA Health is pleased to offer FREE Covid-19 testing for everyone - whether you are experiencing symptoms or not! To make a testing appointment, simply call us at 773-995-6300 to register. We also offer telehealth (Telephone) visits on an ongoing basis as part of our care model! Please have your ID  when you have an appointment. Our drive-up testing center is located at 13336 S. Baltimore Ave, Chicago 60633. 
  • Masks are required in health care settings regardless of vaccination status. All TCA Health Staff and patients are expected to wear a mask at all of our clinic locations. 
  • If you are managing symptoms from home, please do not hesitate to call us if they get worse or change. Call 911 or go to your nearest Emergency Department if your symptoms are severe or life-threatening.

Mobile Student Health Clinic 

The Mobile Student Health Clinic brings primary care services and health education directly to your children. Serving hundreds at various schools on Chicago’s far south side, the van makes care more accessible for Chicago’s youth and eases the burden of care on their parents. Our clinicians provide physicals, vaccinations, and treatment for asthma, diabetes, and other chronic diseases.
